What to Submit with your Passport Application:
* Proof of U.S. Citizenship (original or certified copy of birth certificate, etc)
* Proof of Identity (driver's license, old passport, etc.)
* Two (2x2) Passport Photos (can be purchased at Walmart, Rite Aid,or Walgreens)
* Check or money order made out to the U.S. Department of State:
15 years old or younger............................$80.00
16 years old an older................................$110.00
~~To expedite applications add $60.00 to above fee and an additional fee of $12.72 for the return overnight envelope.
(Also, You will be required to pay for an overnight envelope at the Post Office to send in the application.)
* $25.00 Cash or Money Order made out to the White County Circuit Court Clerk for the filing fee. (Our office does not accept personal checks.)
